This is not an intake or medical form.
This is simply an information form so that you and your Certified TRE® Provider can discuss any issues or concerns the might be important to you prior to participating in the exercises.
It is generally agreed that if you are healthy enough to go to the gym, then you are healthy enough to do TRE®. However; there are modifications and adaptations for people with physical limitations.
It is recommend that you consult your medical doctor prior to beginning any physical activity or exercise program.
Always work within your physical limits.

There are few known contraindications to TRE®. However, the following is a list of cautions or conditions that need to be considered as you go through the TRE® process. If you feel your condition warrants specialized medical skills and knowledge, please discuss this with your Certified TRE® Provider in advance.
If it is determined that you need someone with special skills and/or knowledge in your area of concern in addition to the TRE® skills of a Certified TRE® Provider, then PLEASE take responsibility to locate a TRE® Provider whose scope of practice includes your particular needs before proceeding with TRE®.
IMPORTANT:
If you have any of the following conditions we advise that you only work with Certified TRE® Providers whose scope of practice includes experience in the treatment of these conditions.
Manic/depressive
Bipolar conditions
Schizophrenia
Severe depression
Psychosis
Borderline Personality Disorder
Post-traumatic Stress Disorder
If you have any of the following conditions work within your comfort zone - go slow and be gentle with yourself. Stop if you have any concerns, feelings of discomfort or stress.
Irregular blood pressure (high or low)
Irregular heart conditions (surgery, pacemakers, arrhythmia)
Hypo or Hyperglycemic (medications)
Chronic pain
Epilepsy
If you have any of the following conditions your Certified TRE® Provider will work with you to make sure you do not "overcharge" your body. This could include slowing down the TRE® process and/or skipping the first several TRE® exercises altogether. Because there is a different therapeutic tremor protocol for these conditions, we strongly advise that you only work with Certified TRE® Providers whose scope of practice includes the treatment of these conditions.

Fibromyalgia
Lupus
Lower Back Pain
Do not do TRE® if you:
Have physical limitations such as broken bones and sprains still under physician's care.
Have had recent surgeries and are still under physician's care.
Are pregnant. We have a TRE® Certification Trainer who has expertise in working with pregnant woman and she is developing research is the use of TRE® with this population. Until this research is completed and future TRE® training developed, TRE®, LLC's position is that pregnant woman not do TRE®.

ALWAYS REMEMBER TO DRINK PLENTY OF FLUIDS BEFORE, DURING AND AFTER ANY EXERCISE ROUTINE.
Many people find taking an Epsom salts bath to be soothing after their first few TRE® experiences.
